Photo/Media Usage:

At different times, the First STEP mentoring team will be taking your photograph at various organised events and would like to store them for marketing purposes. For this purpose, please provide the First STEP team with your consent to reproduce one or more photos of you.

Macquarie University has the need to reproduce photographs of students, staff and others to promote the University in its marketing publications, on web sites and in advertising.

I hereby consent to Macquarie University to take or have taken photographs, digital images and/ or audio and/or video footage (the image) of me, and to store the images, make copies of the images and publish the images in any form, in whole or part, and distribute them in any medium including but not limited to print media, the internet, digital and online media and video.

I warrant that I am at 18 years of age and have full legal capacity to
provide this consent.
